  • Manage any medical deterioration on the ward as appropriate. Patients are required to be deemed medically optimised to be transferred for rehabilitation, but due to frailty and comorbidities may require medical input during their rehabilitation admission. Treatment options depend on severity of issue and include both management on the ward and transfer of care to acute hospital ward/hospice.
