We support adults over 18 with autism, dementia, learning disabilities, mental health conditions, Prader-Willi Syndrome (PWS), and behaviours that challenge, through community-based residential care settings and supported living services. The role involves supervising and supporting staff on shifts, delivering personal support and care plans, and contributing to service development. Responsibilities include resolving day-to-day issues, supporting team members, maintaining professional relationships, and supporting individual service users in achieving their goals.
Support staff in maintaining effective relationships with service users, families, colleagues, and external agencies, referring issues to senior staff as needed.
Act as a positive role model, providing supervision and guidance to team members, and supporting their professional development.
Undertake keyworking responsibilities, supporting individuals in planning and reviewing their goals and outcomes.
