Birmingham Community Healthcare NHS Foundation Trust (BCHC) is seeking Level 5 Bank Community Nurses for flexible work in our Early Intervention (EICT) and Community Teams in Birmingham.
EICT is part of a broader Early Intervention programme, which has so far delivered 77,000 fewer acute bed days used, 19,000 fewer non-acute bed days, and reduced system time by 11.5 days per person. The impact has saved £25.8 million for Birmingham to reinvest in health and social care developments.
You will join a team in one of the country's largest health and social care transformations, delivering high-quality care to patients in their homes and other community settings, helping prevent unnecessary hospital admissions and facilitating early discharges.
A full driving license and access to a car with business insurance are essential.
The Early Intervention Community Team is an integrated service that aims to build relationships and break down barriers between organizations, working effectively for the individual, avoiding duplication, reducing delays, empowering staff to make decisions, and building trust across the wider system.
Health and social care professionals across Birmingham collaborate more than ever to help older people and those with complex needs recover faster and live healthier, more independent lives at home.
The results include more people living independently at home, fewer hospital admissions, and faster recovery from crises.
Our teams work with various disciplines within the multidisciplinary team to achieve the best outcomes, ensuring patients' independence and wellbeing.
Responsibilities include assessment, care planning, and relevant referrals as part of holistic health needs care, covering personal care, catheter care, bowel care, pressure area care, palliative care, wound management, diabetes, and other long-term conditions.
You will work as part of an experienced team but also operate autonomously within the community, which may include home visits or clinic appointments.
BCHC provides high-quality community and specialist services within Birmingham and the West Midlands, delivering over 100 clinical services in people's homes and over 200 healthcare settings for adults, children, those with learning disabilities, rehabilitation needs, and dental services.
