We are seeking experienced and compassionate carers to join our Post Hospital Bridging Service in North London. This NHS-funded initiative supports patients who are medically fit for discharge that are awaiting social care provision. Airmid HCAs enable safe hospital discharge and ensure continued care at home until formal arrangements are put in place.
The applicant will be responsible for visiting individuals in their own home managing personal care tasks and medication.
