A leading care provider in East Sussex is seeking experienced and compassionate carers to join their Post Hospital Bridging Service. You will help relieve NHS bed pressures by delivering high-quality, person-centered care for recently discharged patients in their homes. Successful candidates will assist with personal care, medication, and daily living activities while ensuring effective communication with service users and health teams. Full drivers' license and recent experience in an NHS role are essential.

This NHS-funded initiative supports patients who are medically fit for discharge that are awaiting social care provision. Airmid HCAs enable safe hospital discharge and ensure continued care at home until formal arrangements are put in place.
The applicant will be responsible for visiting individuals in their own home managing personal care tasks and medication.
