Job description

The bank hoursavailable will primarily be working within our Day Hospice programme, carryingout care to palliative patients, liaising and referring to other members of theMulti-Disciplinary Team or Primary Care services where necessary. Working with otherhealthcare assistants and volunteers in the immediate Day Hospice team, thepost holder will have great communication and an ability to undertakeadministration linked to the service.The post holder will join our clinical bank providing cover and serviceto our Inpatient Unit, Community Nursing team or Allied Health Professionalteam.

Encourage patients self-ownership of their physical, emotional,spiritual and social needs.

Ensure that the rights and choices of individuals are actively promotedand supported and their beliefs and preferences are appropriately acknowledged.

Provide appropriate clinical care and assistance.

Qualifications and Skills:

A nationally recognised qualification/apprenticeship in health and social care at level 2 or above or the equivalent level of knowledge

Experience of working in a health or social care setting

Numeracy, literacy and IT skills at a functioning level

Ability to work as a team member in multi professional setting

Ability to adapt skills to a palliative care setting
