A community medical organization in Horncastle seeks CEMS Practitioners (Paramedics/Nurses) to provide advanced pre-hospital care and manage diverse patient needs. Asone of our CEMS Practitioners (Paramedics/Nurses), you will provide valuable clinical anddriving skills to primarily support the Doctor or ACP across a range oftaskings. Driving our modern fleet of blue-light response vehicles and workingclosely as part of a two person team, you will become a valued and respectedmember of the CEMS medical team. You will independently, and collaborativelyassist in the delivery of advanced clinical assessment and enhanced medicalcare in the pre-hospital setting. CEMS Paramedics and Nurses will undertake themanagement of patients across all ages and all levels of acuity, ofteninvolving complex presentations, either independently or more commonly as partof a two person team. A large percentage of these patients will be managed inthe community by delivering care beyond the scope of a typical ambulance orprimary care response. CEMS Paramedics and Nurses will be empowered toundertake referrals to hospital and community services, acquire additionalclinical skills and extend their scope of practice. Working closely alongsidecolleagues, CEMS Paramedics and Nurses will have the opportunity to expandtheir clinical skills and medical knowledge whilst delivering first classpatient care. All CEMS Paramedics and Nurses will have already acquired a broadexperience of pre-hospital care and emergency driving, both essential insupporting Doctor and ACP colleagues who may come from a diverse primary andemergency care background.
Deliver a full range of clinical skills and competencies within NMC or HCPC scope of practice, working independently or collaboratively as part of the enhanced pre-hospital emergency care team. Conduct comprehensive clinical assessments, investigations, and treatments for patients of all ages and acuity, in line with LIVES and CEMS SOPs and clinical guidelines. Respond by road to cases allocated by senior clinicians or dispatchers, providing skilled pre-hospital care, scene management, and liaison with emergency services as needed. Support healthcare professionals via telephone or video consultation, ensuring holistic, patient-centred care and enabling informed health choices. Manage patients with varying complexity, delivering emergency interventions, advanced resuscitation, manual defibrillation, ALS drug therapies, and clinical procedures such as IV cannulation, venepuncture, catheterisation, and bladder scanning. Administer medicines, including controlled drugs, within scope and policy, and assist in enhanced diagnostics and advanced treatments in pre-hospital environments according to organisational standards.
The LIVES Community Emergency Medicine Service (CEMS) is an innovative, independently governed pre-hospital care model designed to reduce pressure on ambulance services and emergency departments by delivering advanced care directly in the community. Established in 2020 following Lincolnshire CCGs commission, CEMS provides enhanced assessment, treatment, and interventions for patients who have accessed the NHS via 999. Expert triage and clinical risk management are carried out by senior CEMS clinicians based in ambulance control, who identify cases suitable for a CEMS response. These are attended by one of three multi-disciplinary emergency care teams or three dedicated falls response teams, each operating purpose-built blue-light vehicles equipped with advanced medical technology and specialist moving and handling aids. The service delivers hospital-level care on scene, aiming to prevent avoidable ED attendances and admissions while improving patient experience. Since launch, CEMS has responded to over 52,000 incidents, with approximately 75% resulting in discharge at the scene, avoiding the need for ambulance transport or hospital admission. By combining advanced clinical skills, rapid response capability, and patient-centred decision-making, CEMS enables people to receive safe, effective, and responsive healthcare where they are, supporting both system resilience and patient empowerment

Wherepatients need to be referred into other parts of the NHS, we aim to directpatients to the right speciality first time and enhance their pre-hospitalmanagement by the interventions we perform. You will play a key role inensuring clinicians arrive safely at incidents, and provide skilledpre-hospital care to patients across Lincolnshire. To undertake this role, youwill be an experienced registered Paramedic or Nurse, with demonstratable NHSfront-line experience relevant to the role, and enthusiasm for working in newways of healthcare delivery. You will have no restrictions on your practice inthe UK. Additional experience gained from working in primary care, emergencymedicine, palliative care, in leadership roles or instructing/assessing on pre-hospitalcare courses is desirable.

MAINDUTIES & RESPONSIBILITIES
Operational ClinicalResponse - Patient Care
Undertake the full range of standardclinical skills and competencies within either a NMC Scope of Practice forRegistered Nurses, or HCPC Registered Paramedic, practicing independently andcollaboratively as part of an enhanced pre-hospital emergency care team.
Undertakethe clinical assessment, investigation and treatment of patients experiencingillness and injury across the spectrum of acuity and across all ages, eitherindependently or alongside senior clinicians.
Perform clinical duties in accordance with LIVES and CEMSStandard Operating Procedures and clinical guidelines, recognising ownlimitations and scope of practice.
Respond byroad to patients who have been identified as being suitable for a CEMSresponse by either the senior CEMS Clinician in ambulance control, or dedicatedEMAS LIVES Dispatchers.
Provideskilled pre-hospital knowledge and guidance at complex incidents, demonstrating the ability to organise andmanage the scene until more skilled assistance arrives. This may involveorganising and managing patients, bystanders or liaising with other emergencyservices e.g., Fire and Rescue, Police and Coastguard.
Support otherhealthcare professionals requesting advice from CEMS medical team by providingtelephone, or video consultations according to scope of practice.
Participate inholistic and comprehensive patient assessment, encouraging patients to exercisechoice over their healthcare needs and treatments.
Utiliseappropriate and relevant information to assist patients, carers and advocatesto make informed health choices in line with national guidelines.
Managepatients of all ages, with varying acuity and complexity, where requireddelivering emergency care interventions and resuscitation according to scope ofpractice.
Delivery oradvanced resuscitation, manual defibrillation and ALS drug therapies.
Performclinical procedures, including intravenous cannulation, venepuncture, urinarycatheterisation and bladder scanning in pre-hospital care settings.
Identify,prepare, and administer medicines by all routes correctly in accordance toscope of practice and organisational policy and procedures.
AdministerControlled Drugs, either independently within JRCALC, upon direction of anindependent prescriber, or through a Patient Group Direction.
Assist inenhanced diagnostics and advanced treatments in the pre-hospital environment aspart of the team in line with LIVES policies, procedures and operationalguidelines.
Perform dutiesin often unpredictable pre-hospital care settings where dynamic risk assessmentis needed, and operate in often difficult, remote or challenging conditionswith appropriate support and PPE (Personal Protective Equipment).
Respond tomajor incidents as part of the pre-agreed major incident plans alongside otherpartner agencies.
Liaise withambulance control, relevant hospitals, community services and EMAS colleaguesas required throughout the shift.
Communicateeffectively, demonstrating compassion and re-assurance, particularly whendealing with people who may have difficulty understanding e.g., children,people with learning disabilities, cognitive or mental impairment, anddemonstrate ability to adapt and overcome barriers using interpersonalcommunication skills.
Ensureinformed consent is obtained wherever possible. Where consent cannot be given,CEMS Paramedics and Nurses will be empowered to act in the patients bestinterests according to the Mental Capacity Act and LIVES Standard OperatingProcedures.
Input datainto clinical and administrative databases and systems.
Utiliseappropriate experience, knowledge and skills as part of a two person team inorder to provide high quality patient focused care in the pre-hospital setting.
Dynamicallyassess incident scenes and identify environmental and clinical risks topatients, self and colleagues, taking into appropriate action to minimise thesewherever possible.
Identifysafeguarding concerns and ensure these are reported and documented accuratelyaccording to LIVES and CEMS clinical guidelines and Standard OperatingProcedures.
Completetimely, accurate, contemporaneous clinical records and documentation inaccordance with Caldicott principles, current legislation, informationgovernance and regulatory frameworks.
Ensure anynear misses, operational incidents (actual or potential) are recorded andreported in accordance with LIVES CEMS Standard Operating Procedures.
Liaise withother agencies and participate in, and if required, lead a hot debrief.
Carry outmanual handling of patients in emergency situations following a dynamic riskassessment, and utilising ambulance moving and handling equipment as necessary.
Identify andtake action when other peoples behaviour undermines equality and diversity.
Takeresponsibility for own health and wellbeing, to ensure you are able to performyour clinical role in a safe, effective and professional manner, whilstrecognising any ongoing care of physical and mental wellbeing needs.

Clinical Support Activities
Handoverinformation from shift to shift using CEMS internal systems.
Maintainappropriate standards of cleanliness of vehicles, medical, monitoring andcommunications equipment.
General careand cleaning of personal protective equipment used during clinical operations.
Daily, weeklyand monthly checks of equipment, drugs and monitoring equipment in accordancewith agreed team checklists and standard operating procedures.
Maintenance ofaccurate stock records of consumables and drugs.
Shareknowledge, skills and experience as part of the a close-knit team.
Providesupport and mentorship to internal and external learners, new starters, andstudents on placement.
Controlleddrug documentation in line with LIVES controlled drug procedures.
Undergotraining and induction to undertake shifts within the EOC (Emergency OperationsCentre) at ambulance control, facilitating appropriate allocation and dispatchof LIVES assets and provide remote advice to LIVES, EMAS and other partneragencies and colleagues.
