An exciting opportunity has arisen to support and improve the healthcare outcomes of the Borough. Delivery of these services will be from various locations and mobile units.
Successful applicants will provide healthcare support to residents who cannot access Primary Care during core hours and those who, due to personal circumstances, are unable to access mainstream services when needed.
The role involves all primary care aspects as delivered by mainstream primary care, but in a variety of locations and potentially using different means.
Main duties of the job
- Advising, supporting, and educating patients to prevent disease progression and support development.
- Supporting disease prevention through smoking cessation, NRT therapy, long-term condition reviews, and implementing care pathways.
- Seeking support and guidance from clinical colleagues and other practitioners when necessary.
- Recording patient information such as vital signs, BMI, changes in medical history, and deterioration of chronic diseases, and taking appropriate actions.
- Working with the organization to ensure the best outcomes for those in need.
- Supporting non-medical prescribing during consultations to ensure immediate treatment if required.
- Providing accurate diagnoses and initiating or continuing clinical pathways.
- Ensuring clinical supervision with colleagues and adhering to the NMC code of conduct.
- Undertaking clinical audits as requested.

We are a Primary Care provider within Wigan Borough, supporting general Practice with Enhanced and Extended Access services during evenings, weekends, and Bank Holidays.
Our Bank ANPs support patients who cannot access Primary Care services during regular hours.
Job responsibilities

- Supporting non-medical prescribing to ensure immediate treatment when necessary.
- Providing diagnoses and managing clinical pathways.
- Maintaining best practices through clinical supervision and adherence to the NMC code.
- Undertaking clinical audits as directed.
- Providing guidance to Health Care Assistants supporting your clinic.
Person Specification
Qualifications
- NMC registration
- Non-medical prescriber registration
- Masters degree or working towards one
- Advanced Nurse Practitioner certificate
- Full UK driving licence
