A healthcare organization in England is seeking an Advanced Clinical Practitioner Independent Prescriber. This role involves assessing and managing patient needs, prescribing medication, and leading quality improvement in care delivery. Clinical Practice: Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with undifferentiated diagnoses. Clinically examine and assess patient needs from physiological and psychological perspectives, planning clinical care accordingly. Assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions. Diagnose and manage both acute and chronic conditions, integrating drug- and non-drug-based treatment methods into management plans.
Prescribing and Medication Management: Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs, following evidence-based practice and protocols within scope of practice. Support patient adherence to treatments and provide information on medication regimens, side-effects, and interactions.
Patient Support and Communication: Prioritize health problems and intervene in urgent or emergency situations, including initiating emergency care. Support health promotion and self-care strategies. Manage health needs of women for family planning or sexual health. Conduct minor surgery as appropriate.
Communication Skills: Utilise sensitive communication styles to ensure patients are informed and consent. Effectively communicate with patients and carers, considering cultural and individual communication needs.
Quality of Service Delivery: Work within own competence and professional code. Maintain accurate records. Deliver care aligned with guidelines and evidence-based practices. Participate in quality improvement, clinical audits, and evaluations. Use findings to improve care and investigate incidents or complaints.
Leadership and Development: Take responsibility for personal learning, participate in supervision and appraisal, and adhere to guidelines and standards. Promote a learning environment and serve as a positive role model. Support team development and effective referral mechanisms.
Risk Management and Safety: Manage and assess risks, ensure safe practices, and comply with health, safety, and infection control policies. Monitor work environments for hazards.
Learning and Professional Development: Attend mandatory and additional training, participate in appraisals, and maintain revalidation. Educate patients, carers, and colleagues to facilitate learning and sharing of good practice.
Other Responsibilities: Lead in infection prevention, support quality schemes, and undertake additional duties as required, ensuring compliance with NICE guidelines and other standards.
