All Age Continuing Care(AACC) is a collective term for services provided by the NHS. These servicesassess and provide funding for the care of individuals of all ages to meettheir ongoing health and care needs, in accordance with national eligibilitycriteria. The NHS Sussex AACC Team brings services together to ensure smoothtransitions when peoples needs change and/or they move between eligibility fordifferent services.

Main duties of the job

To provide comprehensive administration support anddelivering excellent customer service for patients and their families or carers, local authority and NHSstakeholders.

The role will encompass varied duties supporting nonclinicaland clinical colleagues, diary management, requesting and collating information, first point of contactfor all enquiries, the preparation of agendas and taking minutes, maintaining detailed and accurate recordkeeping using a variety of mediums and electronic systems, which is core to the role.
