A regional healthcare provider in Greater London is seeking a Senior Physiotherapist/Occupational Therapist to support the Community Independence Service. The Community Independence Service is co-located with London Borough of Hammersmith and Fulham (LBHF) Adult Social Care on King St, Hammersmith as well as within Charing Cross Hospital.
Our service works closely with our acute partners, mainly Charing Cross but also other acute Imperial College Healthcare NHS Trust sites and Chelsea & Westminster Hospital NHS Foundation Trust.
You will be connected with peers across the tri-borough of Hammersmith & Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.
As the senior physiotherapist/Occupational therapist for the CIS Home first team, you will be expected to be a clinician with a broad range of clinical and line management competencies who is able to adapt to a wide variety of circumstances.
You will use evidence based principles, current best practice and specialist skills to assess, plan, implement and evaluate interventions and to optimise patient safety following their discharge from hospital.
You will deliver skilled physiotherapy or occupational therapy care to clients in their own home, based on a thorough holistic and risk assessment of their clinical and personal needs and the agreement of an individualised care plan.
