Transformation Partners in Health & Care (TPHC) is seeking a highly skilled and experienced Project Officer to join the London Co-Occurring Conditions programme.
The programme, established in 2022 and funded until March 2026, aims to improve services and pathways for individuals experiencing homelessness with co-occurring mental health, neurodiversity, and substance use issues. More information can be found here: Programme Details.
The relationship between mental health/trauma and substance use is well established. Despite high incidences of co-occurring conditions among the homeless population in London, access to support remains problematic.
As a Project Officer, you will work within a small, dynamic team across the London Integrated Care System (ICS). Your role will involve collaboration with ICS inclusion health and mental health teams, as well as building relationships with local authorities, voluntary sector, and community stakeholders, especially those involved in substance use and homelessness. You will also contribute to pan-London initiatives within the wider Co-Occurring Conditions team.
You will partner with local stakeholders and Experts by Experience to develop protocols and pathways, aiming to ensure seamless support and treatment access—"no wrong door" approach. You will be part of a team of four leads executing a work programme set by the Pan London rough sleeping drug and alcohol delivery group, driving strategic and operational changes across London.
We seek candidates with experience in stakeholder engagement across London, and a good understanding of or experience with multiple disadvantages, including co-occurring mental health, neurodiversity, substance use, and homelessness/rough sleeping.
The role will be based in a central London office, with travel across London required for meetings and project activities. It is directly line-managed by a Programme Manager within TPHC.
This is a full-time, fixed-term role until March 2026. Internal applicants may be prioritized, but external applications are welcome. The position is also available on secondment (acting up) or rotation basis; seconded staff will return to their substantive role after the contract ends.
