Job summary

University Hospitals Birmingham NHS Foundation Trust is seeking an experienced Heart Failure Clinical Nurse Specialist (Band 7) for a 12-month fixed term/secondment. The post supports hospital and outpatient care across Heartlands, Good Hope and Solihull Hospitals, focusing on assessment, management and ongoing support for adults with heart failure.

The HF CNS will educate patients, prescribe treatments, formulate care plans and coordinate follow-up with cardiologists, GPs and community teams.

Qualifications

• Essential: NMC Registered Nurse; Post-registration CPD in cardiac care; Health assessment module; Non-Medical Prescriber.
• Desirable: MSc in Heart Failure; Mentorship course; Advanced health assessment module.
• Experience as Clinical Nurse Specialist at Band 6 or above with autonomous practice.

As a HF CNS, you core aspect of your role will be patient education, prescribing heart failure treatments, creating treatment plans and arranging follow-up following both inpatient and outpatient reviews, in collaboration with cardiologists, GPs, and wider primary care and community teams.
We are looking for a Registered Nurse with post-registration experience managing heart failure, are an NMP, and have health assessment qualification. You should have, or be working towards, a relevant specialist qualification such as an MSc module in heart failure. Strong clinical decision-making, communication skills, and the ability to work both independently and as part of a wider MDT are essential.

• Registered Adult / Child Nurse on the NMC Register (Field dependent upon role)
• Evidence of post registration CPD / Post registration qualification in cardiac care (appropriate area of speciality or professional knowledge supplemented by specialist experience and short courses to degree / masters level equivalent FHEQ Level 6 /7
• To have health assessment module

Experience
Essential

• Evidence and ability to revalidate as required by the NMC
• Significant years post registration experience as a Clinical Nurse Specialist at B6 or above in defined or associated speciality Extensive experience of working autonomously

• Evidence of achievements and career development in current post and of on-going professional development/competence.
• Demonstrate expert knowledge underpinned by theory and CPD relevant to speciality.
• Experience and enthusiasm for developing educational programmes and teaching / assessing and supervising others in a clinical setting.
