Are you passionate about delivering high-quality healthcare in the community? We are excited to offer a fantastic opportunity for an experienced Band 7 Community Matron or a developmental role for an experienced District Nurse or Community Matron to join our team within Adult Community Services in Surrey.
As a Community Matron, you'll play a pivotal role in the delivery of care within our Integrated Care Teams (ICTs) across Surrey Heath, working alongside multiple internal and external services.
Your expertise and compassion will make a significant difference in improving the health outcomes and quality of life for individuals under your care.
You will need to be an experienced practitioner, with proven clinical abilities in managing patients with complex health needs and long-term conditions, supporting the prevention of avoidable hospital admissions.
This role covers the Surrey Heath locality, and you will be expected to travel across the area as required; therefore, having a driving licence and access to a vehicle is essential.
