Job summary
An exciting opportunity has arisen for a full-time Band 7 Advanced Nurse Practitioner to join our North Birmingham Neighbourhood Mental Health Team, working with our GP partners.
Our community mental health services are well-established teams, and as part of the BSOL community transformation programme, we aim to expand our primary care teams with these roles. This is a unique opportunity to be at the forefront of national changes and developments in community mental health care. The development of our Primary Care mental health team is a vital service evolving in line with the NHS Long Term Plan.
We seek an enthusiastic and dynamic individual capable of working within a multidisciplinary team, collaborating with other professionals to achieve the best outcomes for individuals in the local community served by the team.
You will be responsible for providing mental health triage, assessment, and outcomes-based interventions for service users, including nurse prescribing within the Primary Care Mental Health Team.
You will identify needs and determine the best course of advice, support, or intervention.
You will promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.
Key to this role is your ability to ensure robust communication across BSMHFT and primary care, according to each individual service user's needs, providing support and advice to colleagues within the PCN, including participation in local neighbourhood MDTs.
