Job description

Were looking for a passionate and knowledgeable Band 6 Tissue Viability Nurse to join our dynamic Livingstone Community Tissue Viability Team. You will provide expert wound care and clinical leadership across community settings, supporting patients with complex needs and contributing to high standards of care delivery.

This is a great opportunity for an experienced community nurse or aspiring specialist who is ready to step up into a Band 6 role, offering both clinical autonomy and collaborative working within a supportive team environment.

Main duties of the job
  1. Manage a delegated caseload, prioritising care within the wider Tissue Viability service framework
  2. Undertake holistic patient assessments in a variety of community settings, providing clinical input and guiding care plans
  3. Recognise and escalate complex tissue viability needs, using clinical judgement to ensure timely, appropriate intervention
  4. Work in partnership with multidisciplinary teams and external organisations to support integrated care and pathway delivery
  5. Apply advanced knowledge of wound healing, pressure ulcer prevention, and leg ulcer management to clinical decision-making
  6. Offer guidance and training to healthcare professionals, patients, and carers to improve wound care outcomes
  7. Maintain accurate, contemporaneous clinical records in line with professional and organisational standards
  8. Support service audits, quality improvement initiatives, and the implementation of best practice
  9. Act as a role model for high standards of care, promoting a culture of continuous learning and professional development
