A leading community health provider is seeking an enthusiastic Band 6 Staff Nurse or Therapist for their Community Transfer of Care Hub. The ideal candidate will manage discharge planning, work in a multi-disciplinary team, and require a driving license for travel. We are looking for an enthusiastic and experienced Band 6 Staff Nurse or Therapist, with specialist knowledge and experience in discharge planning and pathways, to join our Community Transfer of Care Hub based at Gravesham Community Hospital.
In the Community Transfer of Care Hub you will manage all referrals into the Discharge to Assess Pathways across Dartford, Gravesend and Swanley. You will work alongside a talented multi-disciplinary team of integrated discharge nurses, therapists, rehabilitation and healthcare assistants and an administrator to assist in discharge planning across acute hospital services.
Gravesham Community Hospital is located a short distance from St Georges Shopping Centre and is surrounded by well-known food establishments and local coffee shops making it an ideal location to be based from.
Whilst you will be based at Gravesham Community Hospital, you will be required to travel to local acute hospitals to provide in-reach support to meet the needs of the service and you’ll also be expected to visit patients in their homes and in care homes to undertake assessments in line with our "Home First" approach. Therefore, adriving license and access to a car for work purposes is essential for this role.
