Job description

We are looking for an enthusiastic and experienced Band 6 Staff Nurse or Therapist, with specialist knowledge and experience in discharge planning and pathways, to join our Community Transfer of Care Hub based at Gravesham Community Hospital.

In the Community Transfer of Care Hub, you will manage all referrals into the Discharge to Assess Pathways across Dartford, Gravesend, and Swanley. You will work alongside a talented multi-disciplinary team of discharge nurses, therapists, rehabilitation and healthcare assistants, and an administrator to assist in discharge planning across acute hospital services.

Gravesham Community Hospital is located near St Georges Shopping Centre, surrounded by well-known food establishments and coffee shops, making it an ideal location to be based from.

While based at Gravesham Community Hospital, you will be required to travel to local acute hospitals to provide in-reach support and visit patients in their homes and care homes to undertake assessments in line with our "Home First" approach. A driving license and access to a car for work purposes are essential.

Main duties of the job

As a Band 6 Team Sister / Therapist, you will:

  • Support and manage the Community Transfer of Care Hub Team on a day-to-day basis, providing clinical/management leadership, support, and guidance.
  • Support the Team Senior Sister and Clinical Service Manager with recruitment as requested.
  • Participate in the induction of new staff, students, bank, and agency staff.
  • Involve patients and carers in planning and delivering community services/placements.
  • Maximise clinical resource use and organise clinical cover where necessary.
  • Monitor and evaluate patient flow, admission, and discharge arrangements, initiating improvements.
  • Support team members in facilitating safe and timely discharge plans.
  • Handle complex cases, including assessing homeless patients to meet statutory responsibilities.
  • Review complex patients requiring 1-1 funding.
  • Support safeguarding issues raised.
  • Develop effective communication with colleagues and agencies like GPs, social care, and nursing colleagues.
  • Take clinical responsibility for placements made by the Community Transfer of Care Hub and resolve related issues.
