The Band 6 Mental Health Screener plays a vital role in the Community Mental Health Team (CMHT), conducting initial assessments, evaluating mental health needs, assessing risk, and supporting care planning. The post holder also contributes to team development, clinical supervision, and workforce planning, ensuring timely access to care and maintaining high clinical standards. Responsibilities

1. Conduct initial mental health assessments using appropriate tools and frameworks.
2. Formulate clinical impressions and care needs based on assessment findings.
3. Assess and manage clinical risk, including safeguarding concerns.
4. Develop and review Dialog plus in collaboration with service users and carers.
5. Deliver therapeutic interventions appropriate to the service user’s needs.
6. Liaise with GPs, social care, and other agencies to coordinate care.
7. Monitor and support service users’ physical health needs.
8. Maintain accurate and timely clinical documentation.
9. Apply relevant legislation including the Mental Health Act and Mental Capacity Act.
10. Promote recovery-focused and person-centred care.
