Responsibilities
  • Caseload Management: Skillfully manage a diverse caseload, ensuring timely and effective delivery of nursing care.
  • Comprehensive Assessment: Conduct in-depth evaluations of patients’ health and develop individualised care plans in collaboration with patients and their support networks.
  • Care Delivery: Administer a wide range of nursing interventions, including wound care, medication management, and palliative care, all while adhering to best practice guidelines.
  • Communication and Collaboration: Forge strong relationships with patients, their families, and healthcare professionals to ensure seamless care coordination.
  • Documentation: Keep accurate and up-to-date records of patient assessments, interventions, and communications to meet legal and professional standards.
  • Safeguarding: Respond appropriately to safeguarding concerns and follow local policies and procedures.
  • Professional Development: Engage in regular supervision and training to enhance clinical competence.
  • Audit and Quality Improvement: Play a role in initiatives aimed at monitoring and improving nursing services.
  • Teamwork: Provide support and mentorship to junior colleagues and contribute to a collaborative team environment.
Qualifications
  • Current registration with the Nursing and Midwifery Council (NMC).
  • At least two years of post-registration experience in a community setting.
  • Proven skills in caseload management and care planning.
  • Proficient in a range of nursing procedures, including wound care and medication administration.
  • Strong communication, interpersonal, and organisational abilities.
  • Capability to work both independently and within a team.
  • Knowledge of pertinent legislation and policies.
  • Valid UK driving licence and access to a vehicle for work purposes.
  • Experience in areas such as diabetes management or palliative care.
  • Community Nursing qualification or similar.
  • Experience mentoring junior staff.
