What you will do:

  • Provide high-quality nursing care and support to patients in the community setting.
  • Conduct comprehensive assessments and develop personalised care plans.
  • Deliver efficient and compassionate healthcare to elderly patients with complex needs.
  • Collaborate with multi-disciplinary teams (MDT) to ensure holistic patient care.
  • Utilize System 1 for patient documentation and communication.
  • Perform occasional home visits to deliver care in a familiar environment for patients.
  • Educate patients and families on health management and disease prevention.
  • Participate in team meetings and contribute to the community health strategy.

Requirements:

  • Previous experience working as a Band 6 Nurse within the NHS is essential.
  • Broad expertise in community nursing, particularly with elderly patients with complex health needs.
  • Experience collaborating in an MDT environment.
  • Proficiency in System 1 is advantageous.
  • A valid driver’s license and access to a car are preferred, although not essential.
