Role and Responsibilities:

• Have specialist knowledge and understanding of strategies and interventions in the treatment of dementia.
• Be proficient in referring to other agencies within the service or referring to other services or agencies where necessary.
• Identify risk factors and act accordingly.
• Provide a communication structure between GPs and other services in LCFT, non-statutory services, to ensure high standards of patient care.
• Provide mentoring and clinical supervision to staff and trainees on placement.
• Actively promote the Trust values within the MAS service and in accordance with the NMC and HPC code of conduct.
• Assist the team manager in ensuring effective assessment, formulation, planning, and monitoring of patient care.

Nursing Qualifications and Requirements:
