HealthJobs Ltd is seeking a Band 6 Physiotherapist to join the Discharge to Assess (D2A) Team in Basildon, Essex. The role offers ongoing work with flexible Full Time/Part Time options and a competitive hourly rate of £26.00.

The Physiotherapist will undertake comprehensive assessments, develop and review treatment plans, evaluate falls risk and environmental needs, and work autonomously within a multi-disciplinary team to support patient discharge and rehabilitation.

Formación

• UK-recognised degree or diploma in Physiotherapy with HCPC registration.
• Post-registration CPD evidence essential.
• Membership of the Chartered Society of Physiotherapy desirable.

Responsabilidades

• Undertake comprehensive physiotherapy assessments for patients referred through the Discharge to Assess pathway.
• Develop, implement, and review individualised treatment and rehabilitation plans to maximise independence.
• Assess mobility, falls risk, environmental factors, and equipment needs; arrange provision as appropriate.
• Work autonomously while managing a complex caseload of frailty and comorbidities.
• Collaborate with multi-disciplinary team including OTs, nurses, social workers, GPs and community services.

What you will do

• Undertake comprehensive physiotherapy assessments for patients referred through the Discharge to Assess pathway within their home environment.
• Develop, implement, and review individualised treatment and rehabilitation plans to maximise functional independence and wellbeing.
• Assess mobility, falls risk, environmental factors, and equipment requirements, arranging provision where appropriate.
• Work autonomously while managing a complex caseload of patients with frailty, long-term conditions, and multiple co-morbidities.
• Collaborate effectively with multi-disciplinary team colleagues, including Occupational Therapists, Nurses, Social Workers, GPs, and community services.

Person Specification

Qualifications

• Degree or Diploma in Physiotherapy recognised within the UK, with current registration with the Health and Care Professions Council (HCPC).
• Evidence of relevant post-registration Continuing Professional Development (CPD) is essential.
• Membership of the Chartered Society of Physiotherapy is desirable.

Registration & Compliance

• Must be an HCPC Registered Physiotherapist.

Experience

• Prior experience as a Band 5 or Band 6 physiotherapist within acute, rehabilitation, frailty, or community services is required.
• Experience in discharge planning and supporting patients with complex health needs is essential.
