Responsibilities

  • Specialist Physiotherapist providing high-quality assessment, treatment, intervention, evaluation and discharge planning within multidisciplinary clinical pathways.
  • Deliver person-centred care across a range of settings, adapting practice to meet complex patient needs and maximise wellbeing and independence.
  • Manage a designated caseload, prioritising referrals and coordinating care in line with service priorities and Trust response times.
  • Work collaboratively with multidisciplinary teams, patients, families, carers and external agencies to ensure a holistic, needs-led approach to care.
  • Contribute to service development and continuous improvement by promoting evidence-based practice, maintaining professional standards and supporting high-quality care delivery.
  • Participate in and lead clinical case reviews, providing specialist physiotherapy advice and recommendations as appropriate. Support effective service delivery through workload management, delegation and care coordination.
  • Supervise Band 5 and unregistered staff, contribute to clinical supervision and deliver training to colleagues and wider services.
  • Maintain effective communication using explanation, motivation, negotiation and sensitivity to support positive patient outcomes.
  • Demonstrate Trust values at all times, use resources responsibly and contribute to team development, quality improvement and safe, effective care.

Job description

Leicestershire Partnership NHS Trust is seeking a motivated and compassionate Band 6 Physiotherapist to join our Community Therapy and Home First Service across Leicester, Leicestershire and Rutland.

You will provide high-quality assessment, rehabilitation and intervention to adults in their own homes, supporting independence and helping to prevent unnecessary hospital admissions.

Managing your own caseload, you will undertake holistic assessments and develop person-centred treatment plans to achieve the best possible outcomes.

Working within an integrated multidisciplinary team alongside Occupational Therapists and Technical Instructors, you will contribute to delivering responsive, patient-focused care and promoting independence.

Successful candidates will be expected to work a range of shifts.
