The NHS is seeking an experienced Physiotherapist to join a well-established Community Therapy service in Leicester. The role involves providing essential support for patients at home, facilitating rehabilitation, and collaborating with a multidisciplinary team. Successful applicants will work autonomously to assess and treat patients, requiring a valid UK driving license and a commitment to excellence in patient care.
We are looking for an experienced and enthusiastic Physiotherapist to join our well-developed, long-standing Community Therapy service. The service offers an urgent community response to support patients in staying at home and avoiding hospital admission, as well as assisting those returning from hospital for ongoing rehabilitation to maximize their potential in their own environment.
Alongside our Urgent Community Response, we also provide community therapy for patients with non-urgent routine needs and input into High Dependency Discharge to Assess (D2A) beds for patients with cognitive impairment, delirium, or non-acute mental health diagnoses requiring 24-hour care and 1:1 support. The aim is recovery, reablement, rehabilitation, and safe discharge home, with long-term care needs assessed accordingly.
Successful candidates will work autonomously, providing assessments and treatment/rehabilitation in patients' homes. We collaborate closely with our multidisciplinary team (MDT) colleagues, holding daily MDT meetings to support patient care.
Applicants must hold a valid UK driving license, have access to a vehicle for work purposes, and be confident driving in all conditions. There may be opportunities across Leicestershire and Rutland.
