Job summary

Sanctuary Personnel is seeking a Band 6 Occupational Therapist for the Discharge to Assess (D2A) Team in Basildon. This is a full-time, ongoing locum role paying £27.00 per hour.

You will be HCPC registered and travel between patient homes as needed, conducting home assessments and coordinating with the MDT to ensure safe discharge planning and continuity of care for diverse case loads across Basildon and surrounding areas.

Qualifications

• HCPC registered Occupational Therapist required.
• Valid driving licence and willingness to travel between locations.

Responsibilities

• Conduct therapy assessments in patients' homes following discharge from hospital.
• Work collaboratively within a multidisciplinary team to provide high-quality care.
• Liaise with patients, families and carers to develop and implement care plans.
• Document assessments and interventions accurately, ensuring compliance with standards.
• Support service development initiatives within the Discharge to Assess framework.
