Key Responsibilities

Deliver specialist Occupational Therapy interventions to patients with recent stroke or neurological diagnoses in community settings.

Independently manage a complex caseload , providing person-centred care using advanced clinical reasoning and evidence-based practice.

Offer professional advice and education to patients, families, carers, and health/social care colleagues.

Ensure smooth care transitions , coordinating with other services to support continuity and patient safety.

Work collaboratively within a multi-disciplinary team , contributing to the achievement of service targets and performance indicators.

HCPC Registered Occupational Therapist .

Proven experience working within neuro-rehabilitation or stroke care .

Strong clinical skills and understanding of best practices in neurological conditions.

Excellent communication, organisation, and collaborative skills.

Driving licence not essential - all qualified professionals are welcome.
