What you will do:

  • Be professionally and legally accountable for all aspects of your work, ensuring that your practice meets high standards.
  • Conduct comprehensive assessments, including those with complex needs, using your specialised clinical reasoning skills.
  • Use evidence-based practices to provide effective and appropriate clinical care.
  • Promote patient independence to prevent hospital admissions and expedite patient discharges by facilitating appropriate care in the optimal setting.
  • Manage your schedule efficiently, delegating tasks as needed and conducting thorough risk assessments.
  • Communicate effectively with patients, carers, and colleagues, adapting your communication style to match patients’ needs.
  • Foster positive inter-agency relationships to ensure continuity and optimisation of patient care.
  • Actively contribute to the development of service aims, objectives, and care models.

To be successful in this role, you need to be HCPC registered with at least two years of relevant experience. While a car driver is preferable, it is not mandatory.
