Job Overview:
We are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at MPFT. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.

Key Responsibilities:

1. Assess, plan, and deliver occupational therapy interventions for patients in the community
2. Work with multi-disciplinary teams to support rehabilitation and recovery
3. Develop and implement tailored care plans to meet individual needs
4. Provide advice and education to patients, families, and carers
5. Manage a caseload, ensuring timely and effective interventions

Key Requirements:

1. HCPC registration as an Occupational Therapist
2. Proven experience in community-based occupational therapy
3. Strong communication and interpersonal skills
4. Ability to work independently and as part of a team
5. Experience in managing complex caseloads is desirable
