What you will do:

1. Conduct assessments for clients in the community to identify their needs and develop appropriate care plans.
2. Implement tailored interventions to enable clients to achieve maximum independence.
3. Collaborate with other healthcare professionals to ensure comprehensive care delivery.
4. Maintain accurate records and documentation as required by the Health Care Professions Council.
5. Travel between sites and client homes, utilizing your driving skills to cover the community area effectively.

To be successful in this role, you will need:

1. A Degree or Diploma in Occupational Therapy and must be registered with the Health Care Professions Council.
2. Previous experience working at Band 6 level is essential, ideally in a community setting, though acute experience is also desirable.
3. A valid driving licence is preferable due to the nature of this community role.
