An exciting opportunity has arisen to provide your mental health expertise working in Primary Care as part of a General Practice team.
This role will be based in South Bristol Primary Care Network (Connexus PCN) at Stockwood Medical Centre and Birchwood Medical Practice. The Mental Health Practitioner (MHP) facilitates early mental health access and support via face-to-face and telephone appointments.
We are redesigning our community mental health services to offer a personalized, preventative, and proactive integrated approach. Primary Care, voluntary services, and AWP are working together to provide a one-team approach for our local populations, and your role as a Mental Health Practitioner is key to the success of this approach.
This post is open to newly qualified social workers. In line with AWP trust policy, all newly qualified social workers (NQSW) will initially be recruited at Band 5, regardless of the advertised role banding. NQSWs are expected to complete their ASYE within 2 years of employment, unless there are exceptional circumstances which may extend this period for a further 2 years.
If appointed to a Band 6 role, progression to this level will be automatic upon successful ASYE completion. If appointed to a Band 5 post, you will be eligible to apply for a subsequent Band 6 vacancy following successful ASYE completion.
