Job description

Band 6 Learning Disability Crisis Intensive Support Team Practitioner

We have an exciting opportunity within the team for registered professionals to join the Learning Disability Crisis Response and Intensive Support Team as a Band 6 practitioner for a full-time post of 37.5 hours per week.

The team provides a service from Monday to Sunday, 8am-9pm, including bank holidays, based at the Agnes Unit. The successful candidates will have the opportunity to discuss flexible working patterns within these hours. Weekends and bank holidays are completed on a rotational basis. Shifts range from 8am-4pm, 1pm-9pm, and 8am-9pm.

You will hold your own caseload and work within a specialist team comprising nurses, healthcare support workers, Occupational Therapists, Speech and Language Therapists, Psychiatrists, and Psychologists, with input from allied health professionals, as part of the wider Learning Disability Service.

The Learning Disability Crisis Response and Intensive Support Team provides support, clinical assessments, interventions, and risk management for people with learning disabilities at risk of inpatient admission or losing their community placement.

This intensive support is based on Positive Behavioural Support and aims to support individuals with mental health needs and a learning disability diagnosis. The service covers Leicester, Leicestershire, and Rutland.

Main duties of the job

  • Triaging new referrals from various sources, including direct referrals from patients, carers, or professionals.
  • Providing clinical assessments, support, planning, and oversight for community patients at risk of admission or placement breakdown.
  • Offering training and advice to carers, providers, and health and social care staff regarding individual needs and Positive Behavioural Support Plans.
  • Providing specialist advice and support to enable individuals to remain in the community and prevent inpatient admissions where possible.
  • Monitoring and supporting pathways, including the Mental Health Pathway and Positive Behavioural Support Pathway, until risks are manageable or reduced, then transferring to LD Community teams for long-term monitoring.

You will need a full driving license and access to a vehicle insured for work purposes.
