What you will do:

  • Manage a caseload of Intermediate Care Unit based patients with complex needs, using specialist skills and evidence-based, patient-centred principles to assess, plan, implement, and evaluate interventions
  • Supervise and train junior staff and rehabilitation assistants, guiding them to excel within the service
  • Provide supervision to students on practice placements, nurturing the next generation of professionals
  • Participate in the planning, delivery, and evaluation of Intermediate Care services within your designated area or team, holding responsibility for defined projects
  • Be an active member of the inter-disciplinary Intermediate Care team, enhancing patient-centred care
  • Develop and provide clinical expertise in a designated specialism and participate in the dissemination of evidence-based practice across the service

Requirements:

To excel in this role, you must be HCPC registered with at least two years of relevant experience. Being a car driver is preferable due to the multi-site nature of the job.
