Avon and Wiltshire Mental Health Partnership NHS Trust is seeking a Specialist Practitioner to join the Incentivised Substance Free Living Team at HMP Eastwood Park. Qualifications

We are seeking a compassionate and motivated Specialist Practitioner to join the Incentivised Substance Free Living Team, part of the integrated Wellbeing Service at HMP Eastwood Park. Working within a multidisciplinary team, you will provide comprehensive mental health assessments and deliver evidence-based, recovery-focused interventions for people with complex mental health, wellbeing and substance misuse needs. You will manage risk, develop and review care and crisis plans, coordinate care, facilitate individual or group interventions and work closely with prison, probation, healthcare, voluntary and community partners. Applicants must hold a relevant health or social care qualification, such as RMN, Occupational Therapist or Social Worker, with current NMC or HCPC registration. You will need substantial post-registration experience in adult mental health services, strong assessment and risk-management skills and knowledge of relevant legislation.

This role is focused on providing appropriate, effective interventions and treatments to people with a range of mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.

The post holder will work autonomously to undertake emergency assessment in a wide range of locations, planning and delivering defined, intensive, specialist interventions in line with the CPA framework to people with severe and enduring mental health needs, who maybe distressed and in crisis.

Where appropriate the postholder will work alongside other services to plan, deliver and review effective “wrap around” intensive interventions to manage risk and prevent hospital admission for those with the most complex needs.
