Job Title: Occupational Therapist – Inpatient
Band:Band 6
Location:London, Greater London
Working Hours: 37.5 hours

An exciting opportunity has arisen for an Occupational Therapist to join our esteemed NHS client’s dynamic team in London, Greater London. This position is available for an immediate start on a short-term contract, offering competitive rates.

Key Responsibilities:

In this role, you will assess and support patients being discharged from hospital to reduce delayed transfers of care across the acute trusts and ensure safe transfer into primary care services. You will conduct thorough patient assessments to ensure safe transfer to the appropriate community setting. Your responsibilities will include identifying and facilitating discharge, preventing unnecessary hospital admissions, and developing and maintaining relationships with key stakeholders, including local authority colleagues. You will establish and maintain robust multi-disciplinary discharge planning arrangements to discharge patients safely, reduce length of stay, delayed transfers of care, and re-admissions. You will proactively coordinate, review, manage, and facilitate discharges into community services as part of the wider integrated discharge hub. Additionally, you will ensure the discharge planning process is responsive to the needs of patients and their carers by working jointly with relevant acute, community, and other external agencies. You will be visible on-site at acute units when required to demonstrate proactive management in discharge planning, empower and support staff to improve standards and develop improved discharge practices into community services, support ward teams in developing processes to ensure all patients have a discharge plan within 24 hours of admission, and work across professional boundaries to maintain positive communication links between community and secondary care staff, social services, private and voluntary sector workers, and allied health professions to promote collaborative working.

Qualifications and Experience:

• Full registration with the relevant professional body (e.g., HCPC)
• Experience and knowledge of discharge practices and community services
• Commitment to Continuing Professional Development (CPD)
• Excellent communication and interpersonal skills
• Ability to work effectively within a multidisciplinary team
