Are you a Band 6 Physiotherapist looking for a rewarding locum role in Essex ? We have an exciting opportunity within a supportive Discharge to Assess team, offering great pay and flexibility!

* Job Title: Band 6 Physiotherapist - Discharge to Assess (D2A)

* Location: Essex

* Duration: Ongoing contract

* Schedule: Monday to Friday, full-time preferred (part-time considered)

* Start Date: ASAP

* End Date: Ongoing

* Rate of Pay: Competitive, high locum rates available

What You'll Be Doing:
Join our team as a Discharge to Assess Physiotherapist and play a key role in supporting patients' safe transition from hospital to home or care placements. In this role, you will:

- Undertake assessment and intervention in the community and care homes under the D2A pathways
- Assess patients' therapy and care needs to promote safety and independence
- Provide interventions aimed at reducing risk of re-admission or harm, supporting patients onto the best pathway for their needs
- Coordinate services by completing person-centred assessments, developing and implementing care and therapy plans
- Assess equipment needs, support discharge planning, and set rehabilitation goals to optimise function and independence
- Work autonomously within professional guidelines and the service's policies and procedures
- Assess for and arrange health or social care packages to meet patients' needs (training provided as needed)

Requirements & Qualifications:
To be successful in this role, you will need:

? Current and valid HCPC registration
? Previous experience in community rehab, discharge to assess, or care home settings
? Strong communication and interpersonal skills
? Ability to work independently and within a multidisciplinary team
? Commitment to delivering high-quality, person-centred care
? Full UK driving licence and access to a vehicle for community visits

Working Hours & Contract Duration:
This role offers a flexible working position , requiring availability Monday to Friday , with flexibility where possible. The contract is ongoing with a strong possibility of extension based on performance and service needs.
