The Role

As a Psychiatric Community Nurse, you’ll provide mental health support and care within the community. Your duties will involve working closely with patients dealing with various mental health issues, including anxiety, depression, and schizophrenia. You’ll conduct assessments to evaluate patients' mental health needs, develop personalised care plans, and provide therapeutic interventions. You’ll also offer counselling, medication management, and crisis intervention, helping patients cope with their conditions and enhance their overall well-being. Additionally, you’ll collaborate with other healthcare and social service providers, support networks, and community resources to facilitate holistic care. You’ll act as advocates for your patients, promoting mental health awareness and reducing stigma associated with mental illness.

Responsibilities

A CPN with the ability to manage an allocated caseload within a Community Recovery Service. Preferably someone with care coordination experience. Support and supervision will be provided. You will be expected to participate in ward reviews, placement reviews, and arrange and attend CPA reviews. Assessments of new referrals, liaising with other services (inpatient services, other community mental health services, outside agencies). You will also be expected to be on the rota to cover duties such as Duty, Depot Clinic, Clozapine Clinic, and to attend and contribute to weekly zoning meetings and monthly business meetings.
