Job Description: Band 6 Community Psychiatric Nurse
Location: Southwark Assertive Outreach Team, Marina House, UK | Pay Rate: £27.00 per hour | Contract Type: Ongoing (Full-Time and Part-Time options available)

Main Responsibilities
- Deliver high-quality community psychiatric care within the Southwark Assertive Outreach Team.
- Provide therapeutic support to service users with complex mental health needs.
- Undertake clinical assessments, including risk evaluations and crisis intervention.
- Maintain comprehensive and accurate clinical documentation.
- Collaborate with a multidisciplinary team to plan and deliver coordinated care.
- Build trusting and supportive relationships with clients and their families through informal engagement.
Person Specification
- Qualifications (Essential): Registered Mental Health Nurse (RMN) or equivalent qualification in Social Work (DipSW/CQSW), Occupational Therapy, Psychology, or another allied health profession.
- Mentorship qualification or equivalent practice placement educator certification.
- Experience: Demonstrated experience in a mental health clinical setting. Competence in clinical documentation and record keeping. Experience working within a multidisciplinary team and supporting complex service users. Previous experience in a community-based role managing a caseload.
- Knowledge, Skills, and Abilities: Understanding of clinical governance and audit frameworks. Knowledge of the Care Programme Approach (CPA) and care coordination. Strong interpersonal skills to build informal, therapeutic relationships with clients, carers, and professionals. Proficiency in mental health assessments, risk management, and crisis response. Effective problem-solving and understanding of social inclusion challenges in mental health.
