Band 6 Community Psychiatric Nurse - Adults Community Mental Health Team

Location: Huntington House, York

Hours: Full-time, 37.5 hours per week (Core hours: 9:00 am - 5:00 pm, Monday to Friday)

Flexible working considered: e.g. 8:00 am - 6:00 pm (4 days), 8:00 am - 4:00 pm, 9:00 am - 6:00 pm etc. Part-time applications welcome.

We are looking for an enthusiastic and experienced Band 6 Community Psychiatric Nurse (CPN) to be part of our vibrant Adult Community Mental Health Team at Huntington House, York. This is a fantastic opportunity to engage with a dynamic team over the next 6 months, providing essential support and care to adults with complex mental health needs.

What you will do:

  1. Provide comprehensive mental health assessments for individuals referred to the service.
  2. Develop, implement, and review care plans in collaboration with service users, carers, and the wider team.
  3. Manage a defined caseload of service users with varying levels of complexity and risk.
  4. Deliver evidence-based interventions to promote recovery and wellbeing.
  5. Participate in multi-disciplinary team meetings and liaise with internal and external professionals, including GPs, social services, housing, and voluntary organisations.
  6. Undertake risk assessments and risk management plans in line with Trust policies.
  7. Support crisis intervention, including the use of CPA (Care Programme Approach) where appropriate.
  8. Maintain accurate and up-to-date clinical records in accordance with Trust and NMC standards.
  9. Supervise and support junior staff and students as required.
  10. Contribute to service development and quality improvement initiatives.

Minimum Requirements:

  1. Registered Mental Health Nurse (RMN) with current NMC registration.
  2. Substantial post-registration experience in a mental health setting.
  3. Proven experience working within a Community Mental Health Team or similar.
  4. Sound clinical knowledge of mental health disorders and treatment modalities.
  5. Excellent communication, assessment, and clinical documentation skills.
  6. Ability to work autonomously as well as part of a team.
  7. Full UK driving licence and access to a vehicle for community visits.
  8. Experience using SystmOne or other electronic patient record systems.
  9. Training in psychosocial interventions or other therapeutic modalities.
  10. Understanding of safeguarding procedures and the Mental Health Act.
