Are you a Band 6 Community Physiotherapist looking for a rewarding locum role in Somerset?
We have an exciting opportunity with great pay and flexibility!

* Job Title: Community Physiotherapist - Band 6
* Location: Somerset
* Duration: Ongoing contract
* Schedule: Monday to Friday, full-time (part-time applicants considered)
* Start Date: ASAP
* End Date: Ongoing
* Rate of Pay: �25-�27 per hour

What You'll Be Doing:
Join our team as a Community Physiotherapist and play a key role in improving the lives of patients recently discharged from hospital. In this role, you will:

* perform comprehensive patient assessments
* create and implement personalised care plans
* deliver evidence-based treatments to optimise patient outcomes
* collaborate with multidisciplinary teams to provide holistic care
* maintain accurate patient records

Requirements & Qualifications:
To be successful in this role, you will need:

* a current and valid HCPC registration
* experience working with elderly and post-acute patients in a discharge-to-assess (D2A) setting
* strong communication and interpersonal skills
* the ability to work independently and within a multidisciplinary team
* a commitment to delivering high-quality patient care

Working Hours & Contract Duration:
This role offers a flexible working position and requires availability from Monday to Friday, with flexibility for part-time schedules. The contract is ongoing with a strong possibility of extension based on performance and service needs.
