Job summary

Sanctuary Personnel is seeking a Band 6 Community Mental Health Practitioner for the Single Point of Access (SPA) in Euston, London. This full-time locum position offers a dynamic, patient-centered environment within a trusted NHS framework.

You will perform timely clinical screening, triage referrals, risk assessments and determine appropriate care pathways, including referral to secondary care where needed.

Qualifications

• NMC Registration is required.
• Experience in community mental health settings is advantageous.

Responsibilities

• Conduct timely clinical screening and triaging of referrals.
• Implement robust risk assessments to guide clinical decisions.
• Assess high-risk or complex patients and provide management advice and interventions.
• Determine suitable pathways including secondary care referrals or community signposting.
• Direct administrative staff to update referral outcomes in RiO.
• Facilitate transfers to secondary care when necessary.
• Operate autonomously within mental health legal frameworks.
• Support colleagues through knowledge sharing and peer support.
• Act as a reliable point of contact for referrers, service users and families.
