An established industry player is looking for a dedicated Band 6 Community Mental Health Practitioner to join their North East Essex Dementia Service. This role is crucial in providing specialized care for individuals with memory problems and dementia, focusing on assessment, treatment, and support. We are committed to providing a responsive, flexible, and evidence-based specialist service for individuals undergoing assessment for memory problems or diagnosed with dementia. Our service offers assessment, diagnosis, treatment, and support, working closely with third-party organizations like the Alzheimer’s Society to support service users and their carers, ensuring effective signposting.
The position is based at the Kingswood Centre. We aim to contribute to service improvement, innovation, and collaboration with healthcare providers to reduce hospital admissions.
Applicants should have professional registration and relevant experience at Band 5 or above.
The Memory Assessment Service provides age-inclusive assessments and diagnoses, while the Dementia Intensive Support Team offers short-term, intensive treatment at patients' residences to prevent admissions. Other community teams support individuals with dementia of all ages to maintain independence through holistic assessments, which include access to community services such as assistive technology, environmental advice, and healthy living initiatives. Education for families, friends, and carers is also provided to better support loved ones with dementia.

The post involves assessing care needs and developing, implementing, and evaluating care programs for patients with dementia in various settings, including their homes, care homes, and hospitals. The role requires collaboration with hospital, community, primary care, social care, and third sector organizations, including Alzheimer’s Society and Age Concern.
The successful candidate will deliver person-centered care, participate in staff and community training, and ensure integrated, multidisciplinary service delivery. Responsibilities also include maintaining high standards of care and fostering effective inter-disciplinary working.
