Job description

We are seeking to recruit a Band 6 Community Mental Health Practitioner in our North East Essex Dementia Service.

We are committed to providing a responsive, flexible, and evidence-based specialist service for individuals undergoing assessment for memory problems or diagnosed with dementia. Our service offers assessment, diagnosis, treatment, and support, working closely with third-party organizations like the Alzheimer’s Society to support service users and their carers, ensuring effective signposting.

The position is based at the Kingswood Centre. We aim to contribute to service improvement, innovation, and collaboration with healthcare providers to reduce hospital admissions.

Applicants should have professional registration and relevant experience at Band 5 or above.

The Memory Assessment Service provides age-inclusive assessments and diagnoses, while the Dementia Intensive Support Team offers short-term, intensive treatment at patients' residences to prevent admissions. Other community teams support individuals with dementia of all ages to maintain independence through holistic assessments, which include access to community services such as assistive technology, environmental advice, and healthy living initiatives. Education for families, friends, and carers is also provided to better support loved ones with dementia.
