Job Responsibilities

1. Hold a case-load within the team and participate actively in the weekly Duty system.
2. Complete Health and Social Care Needs assessments, Access and Assessment tools, and Risk Assessments.
3. Implement Dialog+ interventions, complete Safety Plans and Care Plans, and monitor patients' mental states.
4. Arrange and attend medical reviews with our team of Doctors.
5. Make referrals to relevant care pathways and offer support to identified carers, including signposting for additional support.
