At Forward Trust, we believe in second chances and support individuals in achieving lasting recovery and improved wellbeing. Working within a multidisciplinary team, you will deliver high standards of evidence-based nursing care and specialist interventions to a diverse client group in the community.

Key Responsibilities
  • Clinical Assessment and Care Planning: Undertake detailed biopsychosocial assessments and formulate tailored care plans for individuals with dual diagnosis needs, in line with NICE and clinical guidelines.
  • Treatment Delivery: Administer and monitor opioid substitution therapies (e.g., buprenorphine, methadone) and other pharmacological or psychosocial interventions as appropriate.
  • Integrated Working: Collaborate closely with internal teams, GPs, mental health services, and local authorities to provide joined-up, trauma-informed care.
  • Health Promotion and Recovery Support: Deliver harm reduction advice, relapse prevention strategies, and health education to support sustained recovery.
  • Safeguarding and Risk Management: Identify safeguarding concerns and implement appropriate risk management and escalation protocols.
  • Clinical Leadership: Act as a role model for best nursing practice, support the development of junior staff, and contribute to quality improvement initiatives.
  • Documentation and Compliance: Maintain accurate and up-to-date records in line with NMC standards and Forward Trust policies.

This role does not require prison clearance, but an enhanced DBS check will be required prior to appointment.
