Job summary

Avon and Wiltshire Mental Health Partnership NHS Trust in Bristol is seeking a Band 6 Specialist Intensive Practitioner to cover maternity leave in the South Glos Intensive Team at Kingswood Civic Centre on a fixed term one year.

You will lead shifts, review referrals, carry out core assessments, manage risk and formulate care plans with home treatment or gatekeeping options, and contribute to service development while upholding least restrictive practice.

Qualifications

• Experience as a Band 6 practitioner in mental health services is desirable.
• Ability to work within a multidisciplinary team and coordinate shifts.
• Competence in risk assessment, risk management and care planning.

Responsibilities

• Lead shifts as a shift coordinator and review referrals.
• Complete core assessments and develop risk management plans.
• Deliver least restrictive care, with home treatment or gatekeeping when needed.
• Contribute to service development and participate in multidisciplinary meetings.
• Support safe discharge planning and coordination with partner agencies.
