Job description
Overview

The post holder will work under the direction of the Children’s Community Manager with responsibility for:

  • The assessment of care needs for all patients, ensuring appropriately planned care is both implemented and evaluated.
  • The Nurse is expected to work with minimal supervision and will be designated to take charge and work as a lone worker within the community on a regular basis.
  • The Nurse will provide care at home without supervision to facilitate the prevention of admission or reduction in hospitalisation.
  • The Nurse will act as an effective clinical leader and will ensure the clinical and professional development of all staff for whom they are responsible.
  • Circumstances may arise for the post holder to assist in providing clinical cover in all areas of the paediatric unit.
Responsibilities
  • To assess individual care needs, develop, implement, and evaluate care given to patients in conjunction with the multidisciplinary team.
  • To provide efficient, effective and evidence-based nursing care to children and young people within the community setting.
  • To perform nursing procedures to a high standard in accordance with the Royal Free Clinical Practice Guidelines and policies and to ensure that these policies and procedures are understood and adhered to by all staff within own management responsibility.
  • To be competent in the recognition and management of life-threatening emergencies in children and be able to carry these out in the community setting.
  • To be responsible for highlighting and identification of child protection concerns to the team leader/ child protection lead and auctioning concerns through child protection procedures. To ensure the team adhere to the child protection policies and guidelines.
  • To maintain accurate and comprehensive nursing records, using appropriate documentation and Nursing Information System, and ensure their confidentiality and safe keeping in accordance with the NMC and trust guidelines.
