Overview

Band 6 CHC Nurse Assessor – Northamptonshire area • Day Rate £275 • Contract Duration 3-6 months ongoing • Start Date ASAP - 4 weeks

About the Role

We are seeking an experienced Band 6 CHC Nurse Assessor to join a dynamic team based in the Northamptonshire area. You will play a vital role in facilitating multi-disciplinary assessments and patient reviews, supporting the Continuing Healthcare Specialist Nurse/Clinical Manager, and managing complex cases in line with NHS Continuing Healthcare standards.

This role involves delivering safe, person-centred care while working closely with care providers and internal teams to ensure high-quality service delivery.

Key Responsibilities
  • Facilitate multi-disciplinary assessments and patient reviews in accordance with policies and procedures
  • Manage cases as directed by the Specialist Nurse/Clinical Manager
  • Support Personal Health Budget pathways and ensure compliance with Continuing Healthcare guidance
  • Undertake investigations related to eligibility challenges or disputes
  • Act as Duty Clinician when required
  • Maintain accurate and up-to-date records in line with data protection standards
  • Support the training and induction of new staff members
  • Collaborate with internal and external stakeholders, care providers, and multidisciplinary teams
  • Provide specialist advice and guidance related to CHC services
  • Assist with disputes management, transitional bed processes, SEND, and safeguarding as needed
Candidate Requirements
  • Registered Nurse with current NMC registration
  • Experience in NHS Continuing Healthcare and nurse assessment roles
  • Strong organisational skills and ability to work under pressure
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multi-disciplinary team

Working Hours & Contract Details
  • Full-time role with daily travel around Northamptonshire and occasional countrywide travel
  • Primarily computer-based assessments and patient reviews
  • Contract duration: 3-6 months ongoing
  • Start date: ASAP - 4 weeks
