Band 6 Associate Palliative Care Nurse Specialist

These roles will be to providespecialist palliative and end of life care for babies, children and youngadults in the setting of familys choice hospice, home or hospital. You will beexpected to participate in the 24/7 on call service and available to workflexibly over a 7-day period. As a band 6 post you will have the opportunity towork alongside and gain knowledge and skills from our Specialistmultidisciplinary team and become competent in caring for a diverse group ofpatients from perinatal to young adults and every age in between.

In response to the increasing number of perinatalreferrals, we are seeking to recruit one candidate with a demonstrated interestand expertise in perinatal care.

Main duties of the job

Main Purpose of Post:

To practise and promote specialist palliative care,encompassing a supportive and holistic approach to the baby/child/young personand their families who require palliative or end of life care in the community,hospice or hospital setting.

The post holder will maximise the use of themulti-disciplinary team within the community setting and the multi-skilled teamwithin the hospice to maintain continuity of care for the baby/child/youngperson and their family and in line with the Claire House philosophy.

This role will be a point of contact for symptommanagement, support and advice for babies/children/young people, families andclinical teams from the hospice, community and hospital setting throughout thepalliative care journey from diagnosis to death and through to bereavement.

This role will support the wider Rapid responseleadership team in the implementation of service developments regarding palliativeand end of life care services in all care settings to ensure children andfamilies receive evidenced based care of the highest possible standard.

To provide clinicalleadership, support, and guidance to junior members of the Rapid Response teamand other external professionals who are less experienced or skilled.

To practice as asupplementary and independent prescriber adhering to NMC guidelines

Job responsibilities

The post holder reports to the Palliative Care Nurse Specialist and is accountable to the Integrated Care Service Lead/Nurse Consultant and Director of Clinical Services.

The post holder will have line management responsibilities for Nursing Associate posts within the Rapid response Team.

The post holder works as part of the Rapid Response on call and hospice prescribing teams if qualified NMP.

The post holder works collaboratively with Lead Clinicians, the Hospice, Specialist Palliative Care and Community Childrens Nursing Teams to ensure seamless care is provided to children and their families and to develop excellent channels of communication.

The post holder will influence palliative care through networking, education and training.

Scope of Job

To offer and provide specialist hands on palliative and end of life care in the most appropriate setting according to the choice of the baby/child/young person and family, home, hospice or hospital.

To act as a resource on palliative care issues for all members of the multi-disciplinary team providing supervision, guidance, care and support across all care settings.

To be a key point of contact for all referrals for end-of-life care in all care settings and to co-ordinate, with partner services, care provisions ensuring a timely but safe transfer to preferred place if care.

To co-ordinate the care for a case load of babies/children/young people, ensuring that all they receive safe and effective care and support to meet their identified needs in a timely manner in a setting of choice, using the Band 7 Palliative Care Nurse specialists for guidance.

To facilitate and forge external links with hospitals and community services to ensure that professionals are aware of service provisions that Claire House offer.

Dimensions and Limits of Authority

To work without supervision as an autonomous practitioner ensuring personal accountability for practice and management in accordance with NMCs code: Standards of conduct, performance and ethics for nurses and midwives.

To work collaboratively with other multi-disciplinary professionals from health, education and social care to provide joined up and seamless care.

This post holder will take line management responsibility for identified staff within the team, including appraisals and sickness absence related issues. Appropriate advice should be sought from the Integrated Care Lead/Nurse Consultant or HR.

To provide a 24 hour on call service for telephone advice and home assessment visits using advanced clinical skills to base decisions on.

To be delegated the clinical leadership for the Rapid response team on a shift basis to ensure the safe, smooth running of the service, prioritising care using own initiative.

  • Professional background as a Registered Nurse Child, adult or learning disability
  • ENB 998 or equivalent
  • Demonstrate clinical leadership
  • Community nursing experience
  • Non-medical prescriber (desirable or willing to undertake)

Main Duties and Responsibilities:

  • To work flexibly over a 7-day period to suit the needs of the service.
  • To participate in the 24/7 on call rota for the Rapid Response Service.
  • To participate in the formulation and review polices and procedures.
  • To provide clinical leadership for the Rapid Response team on a shift basis to ensure the service is safe, effective and responsive, prioritising care using own initiative.
