Job summary

HCRG Care Group is seeking a Band 5 Community Staff Nurse to work in Skelmersdale, delivering care in patients’ homes with a family-centred approach. You’ll join a skilled team to promote child and family wellbeing, following NHS standards and Agenda for Change terms.

The role emphasises community health, safeguarding, and collaboration with the SCPHN pathway. You will support diverse families, require NMC registration, and may need driving to visit service users across the locality.

  • Be a registered nurse with NMC registration.
  • Desirable: understanding of health promotion/public health activities.
  • Ability to act independently as well as part of the multi-disciplinary team.

Responsibilities

  • Deliver the healthy child programme at four levels as delegated by SCPHN.
  • Support leadership and delivery of a quality function (experience, supervision per guidelines).
  • Be trauma aware and understand trauma-informed care and Adverse Childhood Experiences impacts.
  • Recognise health needs and refer to SCPHN when necessary.

Job description

The post is based in the Skelmersdale Library postcode WN8 6NL.

Responsibilities
  • Deliver the healthy child programme at four levels as delegated by the Specialist Community Public Health Practitioner (SCPHN)
  • Support leadership and delivery of a quality function (such as, service user experience and undertake supervision as per HCRG Care Group guidelines)
  • To be trauma aware and understand the principles of a trauma informed approach to care and the impact of Adverse Childhood Experiences on children, young people and adults
  • Ability to recognise health needs for individual clients and situations in relation to the health of children, young people, individuals and families, in complex situations, identifying situations where referral for support from SCPHN is necessary

Ideal Candidate
  • Be a registered nurse with NMC registration and a passion for improving the lives of children and families within the local community.
  • Be experienced and have an understanding of health promotion / public health activities is desirable
  • Ability to act independently as well as part of the multi-disciplinary team.
  • Have a valid UK driving licence, with access to a car for work purposes.
