We have an exciting opportunity for you to join our Early Intervention Community Team within Birmingham Community Health Care NHS Foundation Trust, the largest community trust in the country.
Part of a wider Early Intervention programme, which has delivered in the last 12 months: 77,000 fewer acute bed days used, 19,000 fewer non-acute bed days, and people spend 11.5 fewer days in the system. Overall, this has resulted in a £25.8 million saving for Birmingham to reinvest in Health and Social Care developments.
Six health and social care organisations joined forces in the region to deliver this truly integrated service. Alongside BCHC, other partners include Birmingham City Council, University Hospitals Birmingham, Birmingham & Solihull Mental Health Foundation Trust, Birmingham and Solihull CCG, and Sandwell and West Birmingham CCG.
You will be joining a community team amidst one of the largest Health and Social Care transformations in the country, with exciting changes and developments aimed at preventing avoidable hospital admissions, premature admission to long-term residential care, and facilitating early hospital discharge. Birmingham is also a fantastic place to work and live.
This is a real opportunity to work within a self-motivated and aspirational multidisciplinary team (MDT). We are currently looking for someone to dynamically lead and promote a culture of home-first rehabilitation, ensuring people in Birmingham are treated in the right place at the right time, as evidenced by clinical and performance outcomes. You will work multi-professionally, completing assessments and interventions, while ensuring the delivery of high-quality interventions to patients within their own homes and other community settings.

The ideal candidate will be an excellent communicator with a flexible approach, possessing existing links or eagerness to establish links with health and social care partners, and a passion for 'trusted assessor' principles. You will have opportunities to support junior staff and students, as well as lead service development projects and improve patient care.
We offer excellent leadership training through our Inspire programme for staff with line management responsibilities, along with other career development opportunities.
HCPC/NMC registration is required for registered staff roles.
The post holder will be expected to work shifts as required over 7 days per week, in line with service delivery.
